Job Title: Home Care _ Care Coordinator
Salary: £28,000 per Annum
Reports to: Registered Manager
Responsibilities
- To assist the Registered Manager with staff and client reviews
- To assist the Registered Manager with preparing and updating Care plans
- To assist the Registered Manager with Care Consultations
- Daily Update, Communicate, agree with Scheduler Rota changes
- Making sure that the rota has been completed every week and all calls are covered and any changes made aware to carers
- Participate in the on-call rota and complete handover notes
- Ensuring staff rotas meet service users’ needs and aspirations
- Work with Recruitment Team to ensure sufficient current and future staffing levels
- Provide care services to service users in an emergency – arrange emergency cover when required and cover all / any calls employees are reasonably required to carry out visits outside normal working hours
- To ensure telephones are answered promptly and people are spoken to in a polite and respectful manner
- To work alongside the Manager ensuring that all quality standards, complaints, and comments are dealt with promptly and recorded
- Put in place measure to comply with CQC regulations and work to achieve the best care standards possible
- To provide leadership to care staff within the service
- To provide care in accordance with current best practices, according to policy and procedures, agreed standards, legislative requirements and relevant regulations under the direction of the Manager
- To supervise the care delivery within the service
Address and Location
101-6 Chapel Road, Suite 202-204 Chapel House, Worthing, West Sussex BN11 1EX, but you may be required to work from other locations at the discretion of the company and with appropriate notice.
Working Hours
9am to 5pm Monday to Friday, including one in four weeks on call. Monday to Sunday between 5pm to 9am.
Role-specific Duties
- Ensure service users are at the heart of the care delivery and their wishes and preferences enhance their wellbeing
- To supervise staff and ensure that all staff contribute to the efficient running of the service
- To organise and coordinate care delivery in accordance with current best practices, according to policy and procedures, agreed standards, legislative requirements and relevant regulations under the direction of the Manager and within the financial plans
