Job summary

A primary care network in Trowbridge is seeking a Care Co-ordinator (Non-Clinical) to support communication and coordination of care among local nursing homes. The successful candidate will have excellent organisational and communication skills and provide vital administrative support. This is a permanent, part-time role with a supportive working environment and opportunities for development.

Qualifications

  • Proven administrative experience in a busy environment.

Care Co-ordinator (Non-Clinical) – Trowbridge Primary Care Network

Based at: Lovemead Group Practice, Roundstone Surgery, Trowbridge
Hours: 20 per week – negotiable
Salary: Depending on experience
Contract: Permanent

Trowbridge Primary Care Network (PCN) brings together local GP practices to deliver high-quality, joined-up care for our community. We are a friendly, supportive team working collaboratively across practices, care homes, and community services to improve outcomes for our patients.

About the Role

This role will focus on supporting our local nursing homes and learning disability (LD) homes, ensuring effective communication, coordination, and continuity of care for residents.

Responsibilities
  • Liaising regularly with nursing homes and LD homes to arrange and support routine visits and reviews
  • Organising clinic and ward round schedules for visiting GPs
  • Coordinating learning disability health checks and ensuring timely completion of assessments
  • Supporting vaccination programme planning and scheduling for care home and LD home residents
  • Maintaining accurate records and ensuring information is shared appropriately across the multidisciplinary team
  • Providing general administrative support to the PCN care home and LD services
About You

We are looking for someone who is:

  • Highly organised with excellent attention to detail
  • Able to work independently as well as part of a wider multidisciplinary team
  • A confident communicator, able to liaise professionally with care home staff, clinicians, and patients’ families
