Job summary

A leading social care provider in Sunderland is seeking a Care Co-ordinator (Level 2) to join its Supported Living Service. This role is centered on delivering high-quality, person-centred care to clients with physical and complex needs. The successful candidate will implement care plans, review them, and ensure the dignity and respect of all customers. We are seeking to recruit a Care Co-ordinator (Level 2) into our Supported Living Service within the Physical & Complex Care Team who can deliver exceptional standards of person‑centred care and support to customers.

Responsibilities

Your role will be on providing support to customers promoting and developing their life, health, and wellbeing. This may include:

  • Implementing care plans in adherence with our T‑Lap principles.
  • Reviewing care plans.
  • Completing daily/weekly/monthly audits.
  • Support with rota cover.
  • Support to make and attend appointments.
  • Helping with preparation of meals and drinks.
  • Support with domestic household tasks.
  • Administer and record medication.
  • Support with money management.
  • Encouraging community trips and facilitating customers participation.
  • Support with household and personal shopping.
  • Support with meeting friends and family.
  • Support with high standards of nutrition.
  • Adhere to health and safety legislation.
  • Cleanliness and intimate personal hygiene.
  • Opportunities for employment or education.
  • Provide emotional support.
  • Active support techniques and positive behaviour support techniques to increase skill level and achieve set outcomes (training will be provided).
  • To support customer to plan holidays and support them if needed (additional payment).

These principal responsibilities are not an exhaustive list of duties, only a general guide to the role. In consultation with the post holder, the duties may change from time to time to reflect the changing needs of the customer/service.

Compensation and Employment Information
  • Induction which will support you in your own personal development.
  • We can offer 37 hours per week.
  • A starting salary of £28,598 (pro rata if working less than 37 hours per week).
  • Sleep in payment of £43.12 per shift completed (where applicable).
  • Designated rota pattern including daytime, evenings, weekends, and Bank Holidays.
