A leading social care provider in Sunderland is seeking a Care Co-ordinator (Level 2) to join its Supported Living Service. This role is centered on delivering high-quality, person-centred care to clients with physical and complex needs. The successful candidate will implement care plans, review them, and ensure the dignity and respect of all customers. A starting salary of £28,598 per year, alongside sleep in payments and excellent training opportunities, is offered. This is a fulfilling role within a supportive team environment.
We are seeking to recruit a Care Co-ordinator (Level 2) into our Supported Living Service within the Physical & Complex Care Team who can deliver exceptional standards of person‑centred care and support to customers.
