gtd healthcare is one of the largest NHS commissioned Primary and Urgent care providers in North West England, we have excellent full time and part time opportunities for a Care Co-ordinator role to support our patient services in the Greater Manchester area.
Our services operate 24/7 providing vital patient care from Ashton Primary Care Centre, Manchester Royal Infirmary, North Manchester General Hospital, Wythenshawe Hospital and Denton Clinical Hub. We are recruiting for a full time and part time overnight position, up to 37.5 hours a week, Monday to Sunday.
Care Co-ordinators are the first point of contact for health care professionals, patients and relatives of patients accessing NHS urgent and non-urgent care services, they are pivotal in the smooth running of our services and ensure patients are directed to the most appropriate healthcare service or pathway.
Within the Care Co-ordinators service, Driving Care Co-ordinators support our clinical teams with a home visiting service, to bring healthcare professionals to the patient when its required. The successful candidate must hold a full UK driving license, be over the age of 25 and no more than 3 license points to operate our vehicles.
The role of the Care Co-ordinator is fundamental to the smooth running of the out of hours and urgent care service across Greater Manchester.

As a Care Co-ordinator for gtd healthcare, you will be the first point of contact for health care professionals, patients and patients relatives accessing urgent and non-urgent care services across a number of integrated care board areas.
Under the direction of the Senior Care Co-ordinator and clinical shift lead you will register patients onto the relevant clinical system and direct them to the most appropriate service or pathway contributing to a 24 hour, seven day a week Care Coordination Service. This is a multiskilled role and will include call handling, treatment centre and home visiting (where able) elements.
