Role Purpose

The Care Co‑ordinator plays a central role in ensuring the smooth and effective delivery of high‑quality domiciliary and community‑based care services. Responsible for care planning, staff allocation, rota management and communication.

Key Responsibilities
  • Care Assessments and Planning – Support assessments for new and existing service users; maintain and update personalised care plans.
  • Staff Scheduling and Rota Management – Build and maintain rotas; respond to staff absences and ensure continuity.
  • Service Delivery & Operational Support – Ensure shifts and visits run smoothly; support the Registered Manager in daily operations.
  • Quality Assurance and Compliance – Conduct spot checks, audits and client reviews; report incidents and safeguarding concerns.
  • Staff Support and Supervision – Provide communication, guidance and updates to care staff; assist with performance monitoring.
  • On‑Call Responsibilities – Participate in on‑call rota and emergency response.
