Job Summary

Do you love working with people? Are you passionate about reducing health inequalities? Do you have experience co‑ordinating care and advocating for patients? Inclusion Healthcare is looking for a Care Co‑ordinator to join our dedicated practice team and provide high‑quality care to the homeless population in Leicester and Leicestershire.

Job Purpose

To provide proactive, compassionate, and efficient coordination of patient care across Inclusion Healthcare services. The Care Coordinator will help individuals—especially those experiencing homelessness and complex social needs—to access timely, appropriate, and person‑centred support, liaising between healthcare professionals, social care, and community organisations to ensure smooth care pathways.

Key Responsibilities
  • Coordinate and navigate care for homeless patients, ensuring access to health and social services.
  • Arrange and support appointments, home visits and community assessments.
  • Maintain accurate, timely SystmOne records and care plans.
  • Provide trauma‑informed, compassionate support and help patients overcome barriers such as language or digital exclusion.
  • Act as a key point of contact for patients and external agencies, promoting integrated, person‑centred care.
  • Collaborate with multidisciplinary teams and participate in team meetings.
  • Identify patients needing additional support and pro‑actively link them with services.
  • Support administrative and reception work, including letters of support and case documentation.
  • Assist with quality monitoring, reporting and development of patient information materials.
  • Adhere to safeguarding, infection control and health & safety policies, including mandatory training.
  • Support infection control initiatives and contribute to a smoke‑free workplace.
  • Maintain confidentiality and comply with GDPR.
Work Details

Pay: £13,250 a year (18.75 hours/week). Permanent, part‑time (18.75 hours per week). Contract: Permanent. Working pattern: Part‑time. Location: Charles Berry House, 45 East Bond Street, Leicester, LE1 4SX.

Essential Qualifications
  • GCSE grade AC in English and Maths or equivalent.
  • Training and experience in administration or coordination.
  • Experience working with the public.
  • Excellent communication (verbal & written) and strong IT skills.
  • Time management, teamwork, confidentiality, empathy.
  • Personal attributes: compassionate, reliable, proactive, patient‑centred, adaptable, maintains confidentiality.
Desirable Qualifications
  • NVQ Level 2 or above in Health and Social Care, or similar.
  • Experience in primary care, inclusion health or with vulnerable populations.
  • Understanding of trauma‑informed care and NHS systems (e.g. SystmOne).
  • Knowledge of local community services.
  • Passion for inclusion health, cultural competence and reducing health inequalities.
