Inclusion Healthcare is looking for a Care Co‑ordinator to join our dedicated practice team and provide high‑quality care to the homeless population in Leicester and Leicestershire.
To provide proactive, compassionate, and efficient coordination of patient care across Inclusion Healthcare services. The Care Coordinator will help individuals—especially those experiencing homelessness and complex social needs—to access timely, appropriate, and person‑centred support, liaising between healthcare professionals, social care, and community organisations to ensure smooth care pathways.
Pay: £13,250 a year (18.75 hours/week). Permanent, part‑time (18.75 hours per week). Contract: Permanent. Working pattern: Part‑time. Location: Charles Berry House, 45 East Bond Street, Leicester, LE1 4SX.
