A leading UK health service provider is seeking a compassionate Care Act Assessor for a permanent role in Leek. You’ll conduct Statutory Care Act Assessments, collaborate with a dedicated team, and support the personalisation agenda. Essential skills include effective communication and problem solving, along with knowledge of the Care Act 2014. POST: Care Act Assessor - Band 4; 37.5 hours per week
BASE: Cottage Ward, Leek Moorlands Hospital, Ashbourne Road. Leek. ST13 5BQ.
TEAM: Moorlands ASC Community Duty Team.
An exciting opportunity has arisen within the Moorlands Initial Response Team for a band 4 Care Act Assessor. This is a permanent post.
We are looking for an enthusiastic, proactive and innovative individual who possesses excellent communication & organisational skills and has a good working knowledge of the Care Act 2014. The individual must be positive and work flexibly as part of a team with a strong commitment to the personalisation agenda.
There is a requirement to carry out Statutory Care Act Assessments so some experience of direct assessment skills would be preferred. As a Care Act Assessor you will be responsible to a Practice Leader and the ability to prioritise your work load is essential. If you are an excellent problem solver, who is compassionate, with a good knowledge of the Care Act 2014 we would love to hear from you.

On appointment, you will be enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework. The relevant SWLA 'stage' (i.e. stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration. The post holder will be required to take responsibility for their own Continuous Professional Development.
