Working within a multidisciplinary team, you will support patients with chronic cardiac conditions requiring ongoing specialist management. You will build effective relationships with patients and their families, promoting self-management, independence and understanding of their condition.
The role is primarily clinic based, providing specialist assessment and follow-up care. There may also be opportunities to visit patients in their own homes when clinically indicated, ensuring care is tailored to individual needs. As travel is required across the service area, applicants must hold a full UK driving licence. Please note that a vehicle is not provided.
This is an excellent opportunity to make a positive difference to the lives of people living with heart failure, supporting improved outcomes and quality of life.

You will manage a dedicated caseload of patients with heart failure, providing expert assessment, planning, implementation and evaluation of care. Using your specialist knowledge, you will identify patient needs, monitor disease progression and ensure timely interventions that support effective management of complex cardiac conditions.
A key aspect of the role will be delivering personalised health education and lifestyle advice to patients and carers. You will empower individuals to better understand their condition, recognise changes in symptoms and take an active role in managing their health. Through proactive support and monitoring, you will help reduce symptom burden, minimise the risk of further cardiac events and contribute to reducing avoidable hospital admissions.
Working collaboratively with GPs, consultants and other healthcare professionals, you will assist in optimising medical management and treatment plans. You will contribute to service development, maintain accurate clinical records and ensure care is delivered in line with evidence-based practice and national guidance. This role offers the opportunity to develop specialist expertise while providing exceptional care to patients with complex cardiac needs.
