Please note this role is currently only being advertised to staff members who are currently working within the Kent and Medway IICS. An exciting opportunity has arisen for a Cardiac Rehabilitation Specialist Nurse at Maidstone & Tunbridge Wells NHS Trust. Although predominantly based at Maidstone Hospital some cross site working will be required as well as visits to patients home. Applications are invited from a reliable and enthusiastic nurse to join our small cardiac rehabilitation team. We are looking for someone who is passionate about cardiac rehabilitation and is committed to the delivery of excellent care. Experience in cardiology nursing would be helpful, but we are also keen to meet nurses with experience of working in rehabilitation in other specialities. Access to a car for work purposes is essential due to community visits.

Responsibilities
  • Manage a caseload of patients requiring cardiac rehabilitation services as inpatients, in clinics, in patients' homes and during formal cardiac rehabilitation courses (hospital and community venues).
  • Empower patients and their carers to participate in the management of their conditions, thereby aiming to improve the longevity and quality of their lives.
  • Collaborate with the senior cardiac rehabilitation nurse specialist and other team members to provide high quality care.
  • Participate in informal visits prior to application / interview to fully understand the role.
Qualifications
  • Registered Nurse with experience in cardiology nursing or rehabilitation nursing.
  • Strong commitment to patient education and empowerment.
  • Excellent communication and teamwork skills.
  • Valid driving licence and access to a car for work purposes.
