Job summary

East Suffolk and North Essex NHS Foundation Trust is seeking a Specialist Nurse in Cardiology to work within clinics and MDT to provide assessment and treatment planning. The role supports Afib, Valve and Rapid access chest pain clinics, and requires extensive cardiac knowledge with the ability to work autonomously and as a registered prescriber.

  • Educate and support cardiology patients and coordinate with MDT and community teams.

Responsibilities

  • Support delivery of Atrial Fibrillation, Valve and Rapid access chest pain clinics.
  • Be the point of contact for CNS clinic patients, providing ongoing virtual support and liaising with MDT.
  • Collaborate with MDT and community teams to ensure joined-up service delivery.
  • Assess patient needs, plan and evaluate care, and document communications with health professionals.
  • Communicate effectively with consultants and other health professionals; escalate as required.

Job description

To work as a specialist nurse in Cardiology clinics, working as part of the MDT to provide assessment and treatment planning.

This role will support the delivery of Atrial Fibrillation, Valve and Rapid access chest pain clinics.

You will demonstrate an extensive cardiac knowledge with competence in assessment and consultation and registered prescriber as well as the ability to work independent and autonomously.

Provide care for patients in a holistic manner, incorporating physical and psychological care to enable them to live with a cardiac condition.

To provide education and support to the cardiology patients.

To be the point of contact for patients in the CNS clinics to give ongoing support virtually and to assess and liaise with other members of the MDT on their behalf.

To work alongside the MDT to deliver the highest standard of cardiac care and liaising closely with community teams to provide a joined up and effective service delivery.

Engage with the MDT.

This is a fixed term position until 31st August 2027.

Proposed interview date: 27th October 2026.

Manages clinical risk taking responsibility for the assessment of patient needs and the development, planning, implementation and evaluation of care within the ward and post discharge.

Draws on specialist knowledge to act as an autonomous member of the CNS team, and provide specialised management and care of the cardiac patient.

Effectively deliver a high standard of care to the cardiac patient.

Carefully documenting and sharing information with the MDT in a useful and constructive way.

Using methods available to communicate effectively with other health professionals including GP, community nurse specialists and consultants.

To be able to communicate effectively with the consultants and colleagues if there is a patient concern and understand the correct escalation process.
