BHSCT Clinical Nurse Co-ordinator, Care Home Support Team

In order to meet the overall mission of the organisation the post holder will: Work to improve resident care within Nursing Homes by facilitating, enabling and supporting Nursing Home staff in the delivery of high quality. Will support a clinical nurse facilitator to manage a caseload within aligned Nursing Homes. Be a highly specialist practitioner providing clinical expertise in care, advice and support within CHST and multidisciplinary teams. Lead the CHST and work within the multi-disciplinary team to ensure a coordinated approach to safe, effective, person & family centred care across the clinical pathway, and make timely onward referral to other professionals and agencies. Undertake Nursing assessments, provide recommendations for the development of treatment plans, advance care plans or anticipatory care plans for identified needs and work in collaboration with GPs, Palliative Care Consultants/Leads, Multidisciplinary Team. Develop, facilitate and deliver a training program to Nursing Home staff.

Responsibilities
  • Lead in providing a person-centred holistic approach to assess the mental, physical, psychological and social wellbeing abilities and needs of the resident and provide recommendations for the development of treatment plans for identified needs.
  • Act as a specialist expert resource within CHST and Nursing Homes when planning and communicating complex/ highly complex programmes of care to all disciplines and in partnership with residents, their families and carers, to support timely access.
