This Band 6 bank nurse post sits in the Urgent Care Bureau Single Point of Access team at Birmingham Community Healthcare NHS Foundation Trust. The rota runs across seven days with 24-hour cover, so evenings, weekends and bank holidays form part of the pattern. What the job involves

• Provide telephone triage and clinical assessment for patients referred into the SPA service
• Make autonomous clinical decisions on the most appropriate care pathway
• Identify deterioration, clinical risk and safeguarding concerns and elevate as needed
• Assess complex referrals, identify red flags and escalation to emergency services where required
• Work with GPs, acute hospitals, community services, social care and voluntary sector partners
• Coordinate referrals and arrange access to community services
• Deliver evidence-based care and contribute to service improvement
• Support the development of colleagues, students and new staff through supervision, mentoring and education
• Take part in audits, quality improvement projects and clinical governance
• Work a flexible rota across seven days, including weekends and bank holidays
• Give reassurance and expert advice to patients, carers and referrers
• Help with the day-to-day operation of the service when required
