A community healthcare organization in Redhill is seeking a dynamic nurse to join their Responsive Services Team, focusing on providing assessments and treatments in patients' homes. The ideal candidate should be adaptable to change, possess excellent communication skills, and have experience in a community healthcare setting. An excitingopportunity has arisen for a dynamic nurse to join the Responsive ServicesTeam as part of our bank workforce.
The ResponsiveServices team consists of Urgent Community Response (UCR), Virtual Ward(VW),Intermediate Care team (ICT) and the Referral Hub.
ResponsiveServices are an admission avoidance and facilitating discharge service, whoprovide assessment, short-term care and rehabilitation in the patients home.You will complete comprehensive assessments in the patients home andundertake treatments including IV medications, wound assessment, andmanagement. You will work closely with the multi-disciplinary team to promote independence of patients to ensure they can remain in theirown home.
The candidate will be able to adapt to change and work ina busy environment.Excellent communication skills are essential to enable liaison with all our health and social care partners in themanagement of patients discharge from hospital and assessment in the community.
You will also provide professional leadership for designated staff and will have a role in the leadership and line management of the wider multidisciplinary team.
Due to the nature of the role it is essential that applicants are able to drive, have a car suitable for business purposes and have a clean driving licence.

The service runs from 8am-8pm 7 days a week, 365 days a year, the post holder will be expected to work on a rota basis. There are opportunities for both full time and part time hours.

The post holder will work under the directionof the Virtual Ward Matron/Team Lead and in collaboration with ResponsiveServices and also the East Surrey Hospital based Virtual Ward Frailty Team(Consultants, Junior Doctors, Frailty Matrons and Pharmacists). The role willrequire flexibility, working as part of Responsive Services which includes the UrgentCommunity Response, Referral Hub, Virtual Ward and Intermediate Care Teams. Thepost holder will collaborate with Community Nursing, and Specialist Health CareTeams across First Community, liaising with social care and voluntary agenciesto deliver excellent standards of patient care.
