A healthcare service provider in London seeks nurses with community experience for a bank position. Responsibilities include delivering nursing care, assessing patient needs, and working collaboratively in community settings. Candidates must have an RN qualification and a valid driving license. This role offers flexibility with shifts and supports professional development in a diverse environment.
We require nurses with community experience to work in the community nursing team. This is a bank position so shifts would be on an ad hoc basis as the service requires. Core hours are 8.30-16.30 M-F and 8.30-5pm at weekends, how ever you can work between 4-8 hours each day. You will be supported by a team of nurses and support workers lead by an experienced District Nurse.

Visiting service users in their own home, delivering nursing care including medication administration, wound care, end of life care, catheter care and unplanned visits. Working as part of a team you would be delivering high quality nursing care to clients in their own homes.

Professional Duties & Responsibilities
To practice the principles of Community Nursing, maintaining a high standard of professional practice and competence in keeping with YH Standards, Policies and Guidelines. The NMC Code of professional Conduct, Scope of Professional Practice, Standards of Record Keeping.
To maintain a professional portfolio of continuing professional development as required by the NMC and revalidate every 3 years.
To participate in Case conferences and meetings and ensure that all relevant information is exchanged.
To assist the District Nurse Team Leader in participating in the delivery of the NHS Plan, and public health agendas.
To participate in an audit and data entry contributing to the Trust's targets.
To work within the clinical effectiveness framework, Essence of Care.
To access annual individual performance review, identify training needs and develop a continuing professional development plan.
To assist with the induction, mentorship and teaching of newly qualified staff and colleagues. Student nurses.
Be a Practice Assessor on the local register. To participate in working parties, taskforces and clinical governance and in service training as appropriate. Keep up to date with current trends in community nursing, nursing research, clinical guidelines and continuing education programmes.
Maintain high standards of professional practice and competence in keeping with that of the NMC and Trust standards for record keeping.
Communicate clearly and effectively with team members, multidisciplinary team and other agencies providing a seamless service.
Communicate with clients at a time and level appropriate to their needs in a manner that promotes positive health to act as an integral member of the primary health care team.
Maintain confidentiality with sensitive information when supporting clients in line with information governance policies.
Be able to share information when appropriate.
Be able to communicate in a variety of ways which include verbal, written, IT and report writing.
Be able to identify personal levels of stress and access service provided by the YH e.g. counselling services.
