As a Nurse you will work with a good degree of independence, carrying out tasks and responsibilities delegated by the lead clinician and play an important part of the team in meeting the needs of patients in community settings, initially focusing on those referred through the 2‑hour Urgent Community Response (UCR) and later supporting broader referral pathways
Responsibilities
As a Bank Band 5 Registered Nurse you will be a valued member of the Clinical Team, working across organisational boundaries to bring together the strengths of both acute and community services. Your key responsibilities include:
- Coordinating specialist interventions for people who might otherwise require hospital care or who need timely support to leave emergency departments safely; managing your clinical caseload and ensuring smooth coordination of care throughout each patient's journey
- Clinical practice – providing evidence‑based advanced clinical skills to support appropriate patient care under the guidance of a registered clinician. This includes assessing patients, interpreting tests within your scope of practice, planning care, recognising early signs of deterioration, responding promptly, escalating when needed, and documenting your actions clearly
- Management – organising your time and workload effectively, advocating for patients, managing your caseload day‑to‑day, offering advice and treatment recommendations, working closely with clinicians in the Virtual Ward team, and contributing to supervision
- Leadership – serving as a positive role model for colleagues, promoting collaborative working, challenging traditional boundaries in a supportive way while upholding high standards of quality, safety and professional practice
- Supporting the learning environment – demonstrating commitment to your own professional development and supporting the learning of colleagues and student nurses
- Continuous improvement – noticing areas where practice can be strengthened and contributing to improvement efforts within a Just Culture that values reflection and learning
Positions & Scheduling
Requests for duty – staff can request shifts or days off up to six weeks in advance to promote a better work‑life balance and accommodate personal commitments.
The post is based at either Chippenham, Malmesbury or Savernake Hospital and works Monday‑Sunday 8am‑6pm (shorter shifts are considered).
Qualifications
- Registered nurse
- Able to work autonomously and as part of a team
- Experienced in the use of NEWS2 diagnostic tool and when to escalatie
- Contribute to the development of others, supervising and teaching, mentoring, including student nurses
- Previous experience in elderly care, frailty, community services or a relevant acute care speciality
- Excellent written and verbal communication and interpersonal skills
- Ability to adapt to the demands of a constantly changing environment
- Good IT skills
- Work within the NMC code of conduct
- Knowledge of the community environment
- Good communication skills including IT literacy and knowledge of computer systems
- Undertake evidence‑based clinical practice
- Experience of caring for people with long‑term conditions
- Experience of pressure ulcer prevention
- Awareness of self‑neglect, safeguarding and abusive situations to be the patient’s advocate
- Ability to be an effective team member and awareness of team dynamics
- Work within the boundaries of legislation, policies, procedures, and standards
Desirable Qualifications
- Knowledge of clinical systems such as SystemONE

- Clean driving licence, car driver, access to a vehicle for daily use
