Job Title: Locum Pharmacist Integrated Care Home Service

Banding: Band 8A

Location: Nottingham

Start Date: ASAP

Duration: 3 Months

Hourly Rate: 35-37

Working Hours: 20 hours per week (must include Tuesday afternoon)

Job Overview

Pulse is currently seeking a highly experienced and proactive Band 8A Locum Pharmacist to join the Integrated Care Home Service in Nottingham. This is a part-time opportunity for a pharmacist with strong clinical knowledge and experience in medicines management for older adults.

The successful candidate will work closely with a multidisciplinary team including a consultant geriatrician and specialist nurses to support the safe and effective use of medicines for care home residents. Attendance at a weekly MDT on Tuesday afternoons is essential.

Key Responsibilities
  • Conduct medication reviews for new care home residents following assessment by a specialist nurse practitioner.
  • Attend weekly multidisciplinary team meetings with a consultant geriatrician and specialist nurses.
  • Provide expert pharmacological input into patient care discussions and ongoing medical management.
  • Maintain accurate clinical documentation in line with service standards.
  • Apply up-to-date knowledge of medicines management for conditions common in older populations.
  • Contribute to service development and clinical governance initiatives.
Qualifications and Experience
  • Be registered with the General Pharmaceutical Council (GPhC).
  • Have significant experience in clinical pharmacy, ideally within care home or community settings.
  • Demonstrate strong knowledge of pharmacology related to older adults.
  • Be confident in working independently and within a multidisciplinary team.
  • Be proactive, organised and committed to delivering high-quality care.
