Job summary

Qualifications

  • Registered with the General Pharmaceutical Council (GPhC) is required.
  • Significant experience in clinical pharmacy, ideally in care home settings.
  • Strong knowledge of pharmacology related to older adults.
  • Ability to work independently and in a multidisciplinary team.
  • Proactive and committed to delivering high-quality care.

Responsibilities

  • Conduct medication reviews for new care home residents.
  • Attend multidisciplinary team meetings with a consultant geriatrician.
  • Provide pharmacological input into patient care discussions.
  • Maintain accurate clinical documentation per service standards.
  • Contribute to service development and clinical governance.
