Job summary

NHS Professionals Limited is seeking a Band 7 Speech and Language Therapist for a community-based role at Wokingham Community Hospital, Berkshire. The post is bank contract, 2 months, 37.5 hours per week, Mon-Fri, at £28.90 per hour.

You will undertake specialist assessments, formulate treatment plans, maintain clear clinical records, and work within a multi-disciplinary team to improve patient outcomes in a community setting.

Qualifications

• At least 3 years post-graduate experience.
• HCPC registration or equivalent and a recognised SLT degree.
• Full UK driving licence and willingness to travel to different sites.

Responsibilities

• Be responsible for highly specialist assessment, differential diagnosis, formulation of treatment plans, writing of full and clear clinical records and assessment reports reflecting specialist knowledge.
• Provide appropriate specialist intervention, reviewing and patients as appropriate and evaluating the treatment outcomes for each individual.
• Demonstrate good negotiation skills across a range of issues including case management in complex cases.
• Work independently, identifying own training needs and personal objectives through appraisal within an Individual Performance Framework at pre-determined intervals.
• Communicate complex condition related information from assessment to patients, carers, families and members of the multi-disciplinary team/other professions.
