A leading mental health organization is seeking a Senior Practitioner for its Hospital Assessment Team. The role involves coordinating care for young individuals with complex mental health needs, providing critical support and supervision, and fostering a collaborative environment among staff and stakeholders. This senior practitioner position is for the Hospital Assessment Team.
Through leadership and supervision, to coordinate the assessment and care interventions for children and young people with complex mental health difficulties within a CAPA CAMH service framework/caseload. This aims to help individual staff develop increasing skill and confidence with a wide range of health, social care, and wellbeing needs, in setting agreed and realistic priorities with the children, young people, and families we offer a service to.
To contribute to the development of a culture where the personal and positive aspirations of service users are the primary focus of the individualised care plan, while developing and maintaining individual skills in delivering a range of therapeutic interventions. This must be achieved whilst respecting the dimensions of personal choice, diversity, and culture and the benefits which these bring to the therapeutic relationship.
Provision of rapid access to specialist CAMHS advice and support and to enable referrers to deal more effectively with a range of mental health problems.
Management of high-risk young people in the community through intensive support.
Increased detection of mental health problems/needs with improved care, treatment, and promotion of understanding of and confidence in the delivery of mental health care, along with collaboration with all agencies involved in the care of young people and families.
Provision of targeted formal and informal learning opportunities for all staff within the primary care teams to develop their knowledge and capabilities within the mental health component of holistic care.
Provision of information, signposting, and support for young people and families.
