An exciting opportunity has arisen within the North East Essex Dementia Service, working within the Memory Assessment Service alongside supporting the Care home liaison team. The successful candidate will be responsible for the delivery of a high level service for patients experience in Dementia and Frailty. You'll provide expert clinical assessments, diagnostic support, and post-diagnostic interventions for individuals with suspected or confirmed dementia. The successful applicant will work within the existing multi-disciplinary team consisting of nurses, doctors, OT's, Clinical and assistant Psychologists. The role will allow for the development of dementia care pathways and service improvement initiatives within the Memory Assessment Team.

The Memory Assessment Service is an age inclusive service which provides assessment and diagnosis for people experiencing memory problems and other cognitive difficulties such as with language or attention and/or personality/behaviour changes. The role would also include working alongside the care home liaison team in providing support to residents within the care home setting they may also be experiencing memory difficulties or have a diagnosis of dementia. The role encourages collaborative working with relevant parties regarding admission avoidance to both general hospital and older adult mental health inpatients services and ensure holistic care delivery and participate in the continued development of services
The post holder will be responsible and provide effective leadership for a clinical team - providing a high quality service within the resources available within a defined area. They will deliver on specific key areas including;
The post holder will evaluate the strengths and areas requiring improvement and development, and collaborative with the Clinical Manager and the senior team to develop strategies for improvement. The post holder will contribute to facilitating the transformation of community mental health services in line with the North East Essex strategy.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
