We are looking for a registered Band 6 Nurse Team Leader to join our established Community Nursing Team in West Lancashire at Clayton House.
In the role you will work in partnership with our patients and their families to lead in the delivery of patient centred, safe and effective care. The successful candidate will require the ability to demonstrate sound clinical knowledge and skills in the management of patients with complex needs, long term conditions and end of life.
You will have extensive clinical experience and experience of case load management, preferably within the community setting and be able to demonstrate recent and ongoing clinical professional development relevant to this post.
This is a Fixed Term, full time role predominately working Monday - Friday with flexible hours between 7:45am - 6:15pm (and on a band 6 weekend rota)
