We are looking for a registered Band 6 Nurse Team Leader to join our established Community Nursing Team in West Lancashire at Clayton House.

In the role you will work in partnership with our patients and their families to lead in the delivery of patient centred, safe and effective care. The successful candidate will require the ability to demonstrate sound clinical knowledge and skills in the management of patients with complex needs, long term conditions and end of life.

You will have extensive clinical experience and experience of case load management, preferably within the community setting and be able to demonstrate recent and ongoing clinical professional development relevant to this post.

This is a Fixed Term, full time role predominately working Monday - Friday with flexible hours between 7:45am - 6:15pm (and on a band 6 weekend rota)

You will be responsible for the following:

  • To deliver quality care through assessment, planning, implementation, and evaluation.
  • To work in partnership, liaising with health and social care professionals to ensure holistic patient care.
  • To manage a caseload and a team of band 5 nurses and healthcare assistants.
  • Completion of root‑cause analysis on patients.

The successful applicant will have:

  • At least V150 qualified.
  • Community experience adult nursing and able to demonstrate extensive knowledge, analytical and practical skills required.
  • Flexibility in meeting the needs of the service within the community setting.
  • Knowledge in implementing change, leading to service improvements.
  • Experience in case load management.
  • Agree to complete SPQ qualification within first two years of employment.
