A regional mental health service in Kingswood is seeking a Band 6 Specialist ROSE Practitioner to provide assertive outreach and engage with adults facing complex mental health challenges. The South Gloucestershire Recovery Outreach Service & Engagement (ROSE) Team is seeking an experienced and motivated Band 6 Specialist ROSE Practitioner – Care Coordinator to join a dynamic, psychologically informed service dedicated to supporting adults with complex mental health needs.
As part of the South Gloucestershire Recovery Team, you will work with individuals who are hard to engage, have high hospital admission rates, and experience severe and enduring mental illness. With a capped caseload of 15, you will have the opportunity to deliver flexible, creative, and person-centred interventions that improve service users’ quality of life.
You will join a friendly, experienced, and supportive team committed to recovery-focused, holistic care and led by a Senior Practitioner.
Deliver assertive outreach and enhanced engagement interventions to individuals with complex mental health needs.
Undertake comprehensive mental health assessments using tools such as the Cluster Allocation Support Tool (CAST), Recovery Star, and KGV.
Act as a care Keyworker for complex cases and deliver defined therapeutic interventions, including:
Build and maintain effective therapeutic relationships, promoting recovery and social inclusion.
Work collaboratively with families, carers, and multidisciplinary partners, including primary care, inpatient teams, and voluntary services.
Support the development of Personal well being plans, crisis and contingency plans with service users and stakeholders.
Lead, supervise, and mentor other staff and students in the team, contributing to service development and best practice.
Maintain accurate, timely electronic health records and ensure performance and safeguarding standards are met.
