A mental health service provider in Devizes is seeking an experienced Primary Care Liaison Practitioner to conduct assessments and support clients. This role offers a collaborative environment where you can make impactful decisions regarding patient care. Applicants should possess strong clinical judgement, communication skills, and the ability to work independently. Flexible working arrangements with a blend of office and home-based work are available.
Our Primary Care Liaison Team is the front door connecting people with the right support at the right time.
We provide initial assessments and support conversations for people who may require secondary mental health services. Working closely with GPs, voluntary sector organizations, primary and secondary care, we ensure that people receive tailored, compassionate, and effective care.
For example, we may meet someone who we then signpost on to support available within primary care. Or we may think that someone requires a period of support from secondary mental health services and refer them on.
