Job summary

Job description
Overview

We are looking for an experienced mental health clinician to manage a caseload of people from Wiltshire who are either placed in an out of area specialist placement or a forensic setting. The role is to oversee the pathway for the duration of an individual’s treatment whilst placed out of county.

Responsibilities
  • Care coordination, review of placement, discharge planning and repatriation to Wiltshire
  • Advocating for patients and liaising with families
  • Traveling to sites some distance from base and developing relationships with a wide range of organisations, providers and the ICB
  • Undertaking assessments of need and risk and formulating and/or contributing to MDT/multi agency care planning processes
  • Supporting community mental health services in compiling and presenting requests for specialist placements funded via ICB or Local Authority funding panels
  • Providing and presenting statutory reports including MHA Tribunal reports and undertaking reviews of care and producing documentation for funding panels
  • Delivering teaching/training and supervision as appropriate and offering consultation to colleagues in relation to out of county community providers and specialist placements
  • Representing AWP, Wiltshire at local Multi Agency Public Protection Arrangements (MAPPA)
  • Holding and working collaboratively with other agencies across geographic boundaries
Qualifications & Experience

• Mental health clinician with experience managing caseloads in specialist or forensic settings
• Proven ability to coordinate care, conduct risk assessments and contribute to multi‑agency planning
• Strong communication skills, advocacy experience and capacity to work across agencies
