Job summary

Hertfordshire Partnership University NHS Foundation Trust (HPFT) is seeking a Band 6 Social Worker to join the Flexible Assertive Community Treatment (FACT) Team in East & South East Hertfordshire. You will deliver community mental health care, provide clinical leadership, manage referrals and safeguarding, and support adults with mental health issues within a recovery-focused framework.

Qualifications

  • Qualified social worker with registration in HCPC and eligible to practice in the UK.
  • Experience delivering community mental health care to adults.
  • Ability to lead and coordinate multidisciplinary teams.

Responsibilities

  • Provide clinical leadership within the ACMHS and FACT team.
  • Manage a caseload of adults with mental health needs.
  • Participate in referrals, allocations and safeguarding responsibilities.
  • Contribute to service discussions and meetings to improve care.

We have an exciting opportunity for a Band 6 Social Worker to join us in delivering community mental health care to the Flexible Assertive Community Treatment (FACT) Team within East & South East Hertfordshire.

The area is well served with transport links to historic Hertfordshire and its glorious countryside as well as into London and beyond. We are looking for a Social Worker who is passionate about care delivering care to adults with mental health issues.

As a Social Worker in an Adult Community Mental Health Service (ACMHS) you will provide clinical expertise and leadership; participate in the referral and allocation process; safeguard vulnerable children and adults; contribute to case discussions and service related meetings; manage a caseload, provide direct support and advice to Service Users (Adults of all ages) and Carers, within the recovery framework, embracing the individual right to choice and control.
