Job description

The care-coordinator position is an integral part of the Mental Health In-reach Team at Hindley Prison, which delivers high-quality, evidence-based mental health care within the prison environment. Treatment is shaped through needs-driven assessments, personalised care planning, and comprehensive risk management tailored to the prison population.

The team operates through a genuinely collaborative, multidisciplinary, and multiagency model, aiming to reduce distress while promoting autonomy, partnership, and recovery.

The postholder will work closely with colleagues to support the Trust's commitment to providing consistent care and support for individuals with mental health needs in custody. The role demands the ability to respond effectively to psychiatric emergencies and crises across the prison, with an expectation to fulfil the responsibilities of the duty worker role.

Main duties of the job

To take an active role in direct service users care as a key worker.

Lead in the planning, implementation and evaluation of care using as far as possible using evidence-based practice.

To provide specialist guidance to staff relevant to clinical specialty.

Work closely and collaboratively with services users and the multi-disciplinary team to assess, plan, implement and evaluate care.

To ensure that due regard is given to service users customs, values and spiritual beliefs, always acting in a facilitative manner that promotes patients dignity, independence and choice

To promote therapeutic relationships, respecting the uniqueness of each service user, acting in a non-judgmental manner, promoting the rights of the individual at all times.

To actively contribute and participate in therapy groups.

Ensuring that robust structures are in place to facilitate effective discharge planning within the CPA framework.

Ensuring that all practices are within the guidelines set out by the NMC and other professional groups and are in accordance with Trust policies.

Take relevant/ appropriate actions as required to maintain service user's wellbeing/ safety. Promptly report observed changes in service user's condition, accidents, incidents and other important information and take further action as directed.

Maintain accurate records of patient information.

Take responsibility for accessing clinical supervision
