Complete comprehensive physiotherapy assessments for adults with learning disabilities, creating tailored, evidence‑based treatment plans.
Provide expert input on posture, mobility, seating, respiratory care, exercise prescribing, and falls prevention.
Collaborate as a key member of the multidisciplinary team, working closely with OTs, LD nurses, psychologists, and social care professionals.
Offer training, advice, and support to families, carers, and support staff to ensure continuity of care.
Participate in MDT meetings, safeguarding discussions, and case conferences, ensuring a holistic approach to care.
Manage your caseload independently, prioritising effectively and delivering high‑quality, person‑centred interventions.
Maintain accurate, detailed clinical records in line with Trust guidelines, contributing to audits and service improvement activities.
