We are looking to recruit a motivated Band 6 Therapist who relishes a fast paced and ever changing environment to join the Home First Team based at St Peter’s Hospital, Chertsey. The Home First Team covers the front door of the hospital: Accidents and Emergency, and Older Persons Assessment Unit (OPAU) and brings together acute hospital services with Adult Social Care and District and Borough Services while maintaining strong links with the Urgent Community Response Team. The role is supported by senior colleagues and there is a requirement to work closely with other members of the multidisciplinary team to provide timely, holistic and comprehensive therapy assessments, facilitate rapid discharge of medically optimised patients and avoid unnecessary hospital admissions. There will also be expectations for supervision and development of junior staff, contribution to the therapy‑wide training programme and to service improvement initiatives and clinical governance activities. ASPH offers a seven‑day service across some teams and you will be expected to participate in seven‑day working as per service requirements. Flexible working requests will be considered.
