Job description
Responsibilities
  • Provide a high quality ward‑based clinical pharmacy service across rotating rotations including reviewing drug charts for safety and efficiency, drug history taking and medicine reconciliation, medicine supply, patient medication consultations, medicines optimisation services, discharge medication service and other activities associated with medicines management.
  • Participate in departmental and regional education and training programmes and undertake study towards obtaining the Certificate/Diploma in General Pharmacy Practice.
  • Provide pharmaceutical advice on supply and clinical issues to other health‑care professionals as requested.
  • Provide medication‑related advice to patients, focusing on medicines started/stopped or changed in hospital and side‑effect profiles enabling them to obtain maximum benefit from their medicines.
  • Assist in the education and training of pre‑registration pharmacists and other staff in training grades.
  • Supervise the daily activities of junior pharmacy technicians and pharmacy assistants.
  • Participate in all activities of the pharmacy dispensary as required, including dispensing, clinical verification of prescriptions, prescription review, accuracy checking and delivering a high‑quality patient medication consultation service.
  • Participate in late nights, weekends, bank holiday shifts and travel between hospital sites.
