Sanctuary Personnel Limited are recruiting a Band 6 Occupational Therapist for our Rapid Response Team in Redbridge, London. This full-time role delivers urgent community OT assessments and supports safe, timely discharges across Redbridge, Havering and Barking & Dagenham.

The post offers flexible hours and a pay rate up to £27 per hour, depending on NHS and community OT experience. NHS background is essential for success in this role.

Qualifications

• Degree in Occupational Therapy and HCPC registration.
• NHS/community OT experience required.
• Experience in rapid response and community settings preferred.
• Strong clinical reasoning and decision-making.
• Ability to work autonomously in community settings.

Responsibilities

• Conduct urgent OT assessments to prevent hospital admissions.
• Assess home environments and functional ability.
• Implement strategies/equipment to maintain independence.
• Collaborate with MDT for crisis intervention.
• Support safe discharge pathways and reduce delays.
• Coordinate with health, social care, and voluntary services.
• Develop person-centred plans to maximise function.
• Monitor outcomes and adjust interventions.
• Maintain detailed clinical records and contribute to targets.
