What You Will Do:

Lead and manage multi‑disciplinary assessments and regular patient reviews in alignment with CHC standards.

Support the Continuing Healthcare Specialist Nurse and Clinical Manager, ensuring effective delivery of CHC workstreams.

Manage a caseload of patients while maintaining compliance with NHS Continuing Healthcare frameworks and policies.

Promote personalised, patient‑centred care and support the rollout of Personal Health Budgets.

Coordinate and review care packages for individuals eligible for NHS Continuing Healthcare and joint funding.

Investigate eligibility challenges and disputes, ensuring fair and accurate outcomes.

Act as Duty Clinician when required to ensure continuity and high‑quality service delivery.

Maintain accurate and up‑to‑date data systems in accordance with operational policy.

Participate in mandatory and statutory training to maintain NMC revalidation and uphold professional standards.

Collaborate with multidisciplinary teams and external providers to improve patient outcomes.

Engage in ongoing professional development and support new staff through training and induction programmes.
