Job summary

A mental health care provider in Birmingham is seeking a Care Coordinator to manage a caseload of older adult service users. Responsibilities
  • Work alongside the MDT as Care Coordinator of a caseload, responsible for initial assessments and coordination of care needs, planning and implementation of evidence‑based care to a defined group of older adult service users and carers within a defined catchment area/locality, and via a job plan.
  • Provide professional advice and support to other agencies and MDT members and act as Hub Manager in their absence.
  • Act as a role model and resource for junior/less experienced staff.
  • Undertake duty work as allocated.
  • Act as an independent practitioner within a defined community setting, liaising with other professionals as necessary.
  • Manage a defined caseload and provide supervision to others as appropriate.
  • Be responsible for the assessment, planning, implementation and evaluation of nursing care within agreed parameters, and have working knowledge of local, national policies and legislation governing current service provision and audit.
  • Ensure effective communication pathways exist.
  • Promote equality, diversity and rights in accordance with good practice and legislation.
  • Identify and contribute to training needs necessary to provide quality evidence‑based care where appropriate.
  • Participate as an active member of the duty system including the undertaking of emergency assessments.

Recruitment and application process

As part of our safe recruitment practice pre‑employment checks will be undertaken before any appointment is confirmed. You may be required to undergo a Disclosure and Barring Service (DBS) check, and all cases of criminal record history are dealt with on a case‑by‑case basis. Please note, if applicable for your role you will be required to cover the cost of your DBS certificate. This will be deducted from your salary over three months. You will also be required to sign up to the DBS Update Service before commencement with the Trust. The Trust will reimburse this. You will be contractually obliged to maintain your subscription.

All successful applicants will be required to complete the Trust's induction programme. The fundamental (Statutory and Mandatory) training will be in an e-learning format and you will be required to complete training modules online (approx. 9–14 hours depending on post) by the end of your second week with us.

The minimum salary threshold for a Health and Social Care role is £25,000 and for the Skilled Worker route is £38,700. If the starting salary for this role is below this, we will not be able to provide sponsorship.

Overseas applicants applying for entry clearance into the UK must present a criminal record certificate from each country they have resided in continuously or cumulatively for 12 months or more in the past 10 years. Guidance can be found here. Relocation expenses may be reimbursed subject to eligibility. The policy provides for reimbursement up to £8,000.
