This role focuses on delivering Continuing Healthcare (CHC) assessments for individuals with ongoing and complex health needs. Your input will directly impact care planning, funding decisions, and the overall wellbeing of patients, ensuring they receive the right level of support.

Key Responsibilities
  • Undertake detailed CHC assessments, following NHS frameworks and national guidelines.
  • Collaborate with patients, families, and multidisciplinary teams to gather evidence and ensure holistic care planning.
  • Provide clear, evidence-based recommendations and documentation for CHC eligibility.
  • Support the delivery of fair and consistent funding decisions.
  • Maintain accurate records in line with NHS and governance requirements.
Requirements
  • Registered healthcare professional (Nurse, AHP, or equivalent qualification).
  • Previous experience in CHC assessments, case management, or NHS commissioning.
