Job Summary

The successful applicant will work with the existing team (consisting of nurses, doctors, occupational therapists, clinical psychologists and assistant psychologists) in Wiltshire to assess eligibility for further memory clinic assessment, provide comprehensive initial assessments and diagnoses, utilising evidence based cognitive assessment tools. The post holder will also be involved with monitoring medication for dementia and the current project of developing shared care with Wiltshire GP's. Excellent liaison skills are needed to ensure the smooth running of the service. You will need a full driving licence and transport.

Responsibilities
  1. Provide specialist memory assessment for referred service users using specified assessment tools to an agreed standard.
  2. Develop and maintain effective therapeutic relationships with service users and significant others.
  3. Work in partnership with service users and carers, providing them with relevant information, signposting them to relevant agencies and assisting them to make choices and plans for their future care.
  4. Provide a specialist service in a variety of settings, managing your own caseload and delivering interventions based on best available practice.
  5. Maintain accurate and up‑to‑date records of casework, ensuring compliance with all relevant standards and Good Practice Guidelines.
  6. Ensure the monitoring and evaluation of the service by collecting and recording high quality and timely data using electronic and paper systems as required.
  7. Attend clinical and team business meetings as necessary.
  8. Participate in your own supervision and appraisal, working with your line manager to set and review objectives and to identify and meet your training and professional development needs.
  9. Maintain professional registration and abide by the NMC code of conduct.
  10. Keep up to date with developments in dementia care, ensuring your practice is evidence based and meets all relevant standards.
  11. Supervise other staff as required.
  12. Contribute to the provision of training for students, working with colleagues to ensure the service meets relevant standards.
  13. Perform other duties as agreed with line manager.
