Job description

Job Overview

An exciting opportunity has arisen to join the Looked After Children Service at Leicestershire Partnership NHS Trust, where you will play a key role in improving the health and wellbeing of some of the most vulnerable children and young people within our communities.

Our Looked After Children Health Team is a friendly, supportive and experienced team of Band 5 and Band 6 Nurses and Care Navigators dedicated to improving outcomes for children and young people in care. Through strong partnership working and a child-centred approach, we undertake statutory health assessments, promote health and wellbeing, provide advocacy, and coordinate support to ensure children and young people receive the best possible care and opportunities to thrive.

As a Band 6 Nurse, you will provide specialist health assessments, care planning, health promotion, advocacy and ongoing support for children and young people in care. Working collaboratively with colleagues across social care, education and partner agencies, you will contribute to delivering high-quality, child-centred care and improving health outcomes.

We are looking for an experienced and motivated nurse with a passion for reducing health inequalities and promoting the voice of children and young people. You will be supported by an experienced multidisciplinary team and will have opportunities to further develop specialist skills in safeguarding, trauma-informed practice, attachment, emotional wellbeing and leadership.

This role is ideal for a nurse who enjoys autonomous working, professional challenge and making a meaningful difference to the lives of children and young people.

A full UK driving licence and access to a car during working hours are required.

Job Description Summary (Main duties and responsibilities)

As a Looked After Children Nurse, you will manage a caseload of children and young people in care, undertaking Review Health Assessments, developing SMART health plans and working collaboratively to ensure identified health needs are met. You will act as a trusted advocate, support transitions into adulthood and contribute to safeguarding, statutory reviews and multi-agency planning.

This is an exciting opportunity to increase your experience, skills and knowledge working with children in care, with a particular focus on supporting Unaccompanied Asylum-Seeking Children (UASC) to reach their potential and meet their health outcomes.

The successful candidate will demonstrate strong communication skills, emotional resilience, professional curiosity and a commitment to delivering high-quality, child-centred care that enables every young person to achieve the best possible health and wellbeing outcomes.
