Job summary

HCRG Care Group is seeking a Band 6 Health Visitor to lead a health visiting team and deliver the Universal Partnership Plus reach of the Healthy Child Programme. You will assess needs, plan tailored interventions, and work collaboratively with paediatricians, speech and language therapists, and dietitians to achieve the best outcomes for children.

Qualifications

• Essential SCPHN qualification and registration with NMC.
• Experience leading health visiting teams and multi-agency collaboration.
• Experience delivering interventions to safeguard children and improve outcomes.
• Leadership and innovative practice in public health nursing.
• Experience in line management and developing teams.
• Experience presenting policy/outcomes to commissioners and partners.
• Ability to work with electronic health records and IT systems.

Responsibilities

• Public health lead for the health visiting skill mixed team within the defined population/locality area.
• Deliver Universal Partnership Plus reach of the Healthy Child Programme and assess health needs.
• Collaborate with key partners to implement interventions and manage child protection plans.
• Develop professional relationships with families and other agencies to promote wellbeing.
• Signpost to partner agencies and contribute to multi-agency assessments.
• Lead caseloads autonomously and supervise other health visitors as needed.
• Understand safeguarding and clinical governance to protect vulnerable families.
