  • Independently manage and prioritise your caseload.
  • Provide seamless transition pathways from inpatient to outpatient status.
  • Collaborate closely with the multi-disciplinary team.
  • Actively participate in service development for stroke, long-term neurological conditions, and older people services.
  • Deliver training and teaching to carers, other health professionals, and students.

You must be registered with the HCPC and hold a valid driver’s licence as you’ll need to travel to various community locations.
