Job Overview

Community Liaison Nurse - Admission Prevention Team. An exciting opportunity has arisen within the Admission Prevention Team based at the Royal Sussex County Hospital for an experienced nurse. The aim of the team is to triage and assess patients that come into the emergency department by ambulance, with a view to preventing admission to hospital.

Key Responsibilities

The post holder will work within the A&E department at Royal Sussex County Hospital. They will act as an ambassador for Sussex Community NHS Foundation Trust, supporting the front door of A&E in preventing hospital admissions. Utilizing knowledge of community services and clinical skills they will work closely with A&E staff to ensure seamless, safe and effective discharge plans for patients to return to their usual residence as soon as possible, preventing admission to hospital.

Key tasks include:

  • Using data from incidents, feedback, and observation to identify potential improvements or changes within the service supporting effective safe discharge.
  • Supporting&E staff in discharge decision-making and management of discharging patients into the community and community beds.
  • Assisting with induction of new staff.
  • Attending MDT meetings and supporting A&E staff with complex discharge functions as required.
  • Providing training and education to A&E staff about community services support and discharge management processes.
  • Ensuring records and data are maintained timely in line with information governance and data sets required for the service.
  • Proactively identifying patients already known to community teams and ensuring discharge back to community planned as soon as clinically safe.
