Job description
Overview

Solar is a partnership between Birmingham and Solihull Mental Health NHS Foundation Trust, Barnardo’s and Autism West Midlands providing emotional wellbeing and mental health services to children and young people aged 0‑19 in Solihull. The team offers timely access to appropriate support based on individual needs, prioritising resilience, partnership and co‑production.

Responsibilities
  • Provide nursing and therapeutic interventions, including assessment, coordination of care needs, planning and evidence‑based implementation for children, young people and families.
  • Ensure a positive experience for families accessing Solar.
  • Provide high‑quality assessment for referred patients, risk assessment and management, formulation of need, information gathering and offer advice and guidance to children and young people and their support networks.
  • Manage a defined caseload and act as a care co‑ordinator within the service.
  • Work with partner agencies (statutory and third sector) to coordinate care of children and young people in Solihull.
  • Ensure the most relevant care pathway and care plan is devised and provide seamless transition into other Solar teams and practitioners.
