As a key member of our team, you will be responsible for assessing, planning, implementing, and evaluating holistic nursing care for a diverse caseload of patients, promoting independence and well‑being. You will work autonomously, managing your own caseload, whilst also collaborating closely with a multidisciplinary team including GPs, social workers, therapists, and other healthcare professionals. A Band 5 Community Staff Nurse provides holistic, patient‑centred care in homes. Duties include comprehensive assessment and individualised care planning; diverse clinical interventions like wound care, medication administration, catheter care, and palliative support; autonomous caseload management, prioritising visits and documenting accurately. You communicate effectively with patients, families, and multidisciplinary teams (GPs, OTs, Social Workers) for integrated care.
Applicants who are not UK or Republic of Ireland nationals will require sponsorship to obtain either a Health and Care Visa or a Skilled Worker Visa unless they already hold permission to work via another route. Prospective candidates should self‑assess their eligibility for sponsorship by reviewing the UK Government’s guidance. For eligible candidates, the Health and Care Visa offers reduced application costs and exempts payment of the annual Immigration Health Surcharge.
