To provide high-quality, evidence-based nursing care to patients within their homes and community settings. As a core member of the multidisciplinary team, you’ll focus on promoting independence, wellbeing, and preventing unnecessary hospital admissions — ensuring continuity and excellence in care across Sussex.
Conduct holistic nursing assessments and create personalised care plans.
Deliver a wide range of nursing interventions, including wound management, medication administration, palliative care, and long-term condition management.
Monitor patient progress and elevate clinical concerns appropriately.
Support seamless hospital discharges and ensure effective communication between healthcare teams.
Promote health education and empower patients to manage their conditions independently.
Maintain accurate clinical documentation in accordance with NMC and Trust guidelines.
Adhere to infection prevention and control standards to deliver safe, high-quality care.
Registered Nurse (Adult) with current NMC registration.
Valid UK driving licence and access to a vehicle for community visits.
Experience in community or acute healthcare settings.
Excellent communication and interpersonal skills.
Ability to work both autonomously and within a multidisciplinary team.
Post-registration experience in community nursing.
Expertise in wound care, long-term conditions, or palliative care.
