A regional health care organization is seeking a Band 5 Community Nurse to join the Community Nursing Team in Lampeter. The successful candidate will be responsible for managing a delegated caseload, providing high-quality care to patients in their homes, and ensuring effective communication with families and other agencies. Applicants should be registered health care professionals, with bilingual communication skills being an asset. This position offers permanent employment with an 8am-8pm service covering weekends and holidays.

We have an exciting opportunity for a Band 5 Community Nurse to join our Community Nursing Team in Lampeter/Llanybydder. The role will allow you to support the management of a delegated caseload within the Community Nursing Team, providing high quality and complex care management to patients in their own homes, with the guidance and support of an experienced District Nurse Team Leader.
The post holder will ensure high standards of care are delivered to patients and their families/carers within the home or clinic environment. This includes liaising with staff, patient/carers and other agencies/organisations as required to ensure appropriate delegation of duties and risk management, whilst ensuring effective communication between those involved in the care package.

This is a permanent position, worked over a 7‑day period including weekends and bank holidays, covering an 8am‑8pm service.
