About the role

We have an exciting opportunity for a Band 5 Community Nurse to join our Community Nursing Team in Lampeter/Llanybydder. The role will allow you to support the management of a delegated caseload within the Community Nursing Team, providing high quality and complex care management to patients in their own homes, with the guidance and support of an experienced District Nurse Team Leader.

The post holder will ensure high standards of care are delivered to patients and their families/carers within the home or clinic environment. This includes liaising with staff, patient/carers and other agencies/organisations as required to ensure appropriate delegation of duties and risk management, whilst ensuring effective communication between those involved in the care package.

Key responsibilities
  • Support the management of a delegated caseload within the Community Nursing Team.
  • Provide high quality and complex care management to patients in their own homes.
  • Ensure high standards of care are delivered to patients and their families/carers.

  • Ensure appropriate delegation of duties and risk management.
  • Maintain effective communication within the care package.
Qualifications and requirements
  • Band 5 registered Health Care professional.
  • Ability to travel between sites in a timely manner.
  • Ability to speak Welsh is desirable; English speakers also welcome.
  • Desire to uphold bilingual policy and increase Welsh language communication.
Working hours and location

This is a permanent position, worked over a 7‑day period including weekends and bank holidays, covering an 8am‑8pm service.
