This part‑time Band 4 role is ideal for someone passionate about supporting adults living with severe mental illness to regain stability, independence, and an improved quality of life. As a key member of our multidisciplinary team, you will work closely with service users, placing their needs and aspirations at the heart of every intervention, and collaborate with nurses, occupational therapists, psychologists, peer workers, and external partners to deliver person‑centred, community‑based care.
Responsibilities
- Provide care coordination for an allocated caseload of service users, ensuring their needs are assessed, reviewed, and met through personalised care plans.
- Deliver goal‑focused, recovery‑oriented interventions to support individuals in developing skills, confidence, and independence.
- Facilitate and contribute to group work and therapeutic clinics, supporting service users to engage in structured activities that promote wellbeing.
- Work collaboratively with internal team members and external partners to ensure high‑quality, person‑centred care.
- Maintain accurate, timely documentation in line with professional and organisational standards.
- Participate in multidisciplinary discussions, contributing to risk assessments, care planning, and holistic treatment approaches.
- Share your knowledge, experience, and skills with colleagues, students, and new staff, supporting a culture of learning and continuous improvement.
- Promote service user involvement and uphold values of dignity, respect, and empowerment in all aspects of care.
- Hold a small caseload of service users, completing assessments, developing person‑centred care plans, and regularly reviewing progress.
- Provide structured, recovery‑oriented support that helps individuals build confidence, develop coping strategies, and work toward personal goals.
- Contribute to the running of therapeutic groups, psychoeducation sessions, and community clinics, encouraging service user participation and engagement.
- Liaise effectively with internal team members and external health and social care providers to ensure coordinated, holistic care.
- Participate in risk assessments and implement risk management plans, ensuring safety and wellbeing remain central to all work.
- Maintain accurate, up‑to‑date documentation in line with organisational and professional standards.
- Engage in supervision, reflective practice, and training opportunities to enhance skills and support career progression.
- Provide guidance and support to colleagues, students, and new staff members, contributing to a positive learning environment.
- Promote service user rights, involvement, and independence, ensuring dignity and respect are upheld in all interactions.
