Responsibilities

• Undertake structured medication reviews and improve medicine optimisation.
• Provide professional specialist medicines advice to healthcare professionals.
• Work within Pharmacy Hub alongside the Pharmacy Team.

Skills

Confident in own professional judgment and excellent decision-making skills

Ability to identify risk and escalate appropriately

Evidence of working autonomously or as part of a team

Education

Master's degree in pharmacy or equivalent

Registration with the General Pharmaceutical Council (GPhC)

Postgraduate Diploma in Clinical Pharmacy (or equivalent)
