Job summary

West London NHS Trust invites applications for a Community Mental Health Practitioner in Ealing Acton MINT on a fixed-term basis (1 year initially with potential extension). You will work within a multidisciplinary team and have autonomy to manage a caseload of patients with varied mental health conditions.

Qualifications

• Prior experience of working across a range of mental health diagnoses/need
• Knowledge of and experience in delivering a range of brief interventions to patients with mental health needs;
• Good written and verbal communication skills and the ability to use these to strengthen local relationships with the local MINT Teams

Responsibilities

• Work in a multidisciplinary Team with autonomy to manage a caseload of patients with varied mental health conditions
• Collaborate with GP practices and other community partners to provide integrated, place-based care
• Dissolve barriers between mental health and physical health, social care, VCSE organisations and local communities to deliver coordinated care
