Job summary

Locum Ready is seeking an Advanced Clinical Practitioner to support regional community health and primary care services around Snodland. This flexible, sessional role is ideal for ACPs with autonomous clinical assessment and the ability to respond to urgent patient demand.

You will work with established clinical teams delivering ACP-led sessions, managing acute and minor illnesses, triaging patients by telephone or face‑to‑face, and prescribing safely within your scope.

Qualifications

• Advanced Clinical Practitioner (Level 7) qualification.
• Clinical background in nursing, paramedic science, AHP or similar.
• Independent Prescribing (V300) desirable.
• Experience in GP, urgent care or community health services.
• Ability to work independently in varied clinical environments.

Responsibilities

• Deliver ACP-led sessions within community and primary care settings.
• Managing acute, undifferentiated and minor illness presentations.
• Supporting urgent patient demand through telephone or face‑to‑face triage.
• Prescribing safely within scope of competence.
• Managing clinical actions including results, medication reviews and follow-up tasks.
• Recording accurate and clear digital documentation.
