Qualifications

  • Registered Nurse, Paramedic, or relevant healthcare professional with NMC/HCPC registration.
  • Meets the standards for Advanced Clinical Practice (ACP) at advanced level.
  • Qualified Independent Prescriber.
  • Experience working in primary care as an ACP.
  • Meets professional revalidation requirements.
  • Master’s degree in Advanced Clinical Practice.
  • Experience in prescribing and conducting medication reviews.
  • Disclosure Barring Service (DBS) check.

Responsibilities

  • Assess, diagnose, and manage patients with acute and chronic conditions.
  • Order, interpret, and act on investigations.
  • Prescribe and review medications independently.
  • Perform minor procedures.
  • Develop and implement individualised care and treatment plans.
  • Manage long-term condition clinics.
  • Provide health promotion and lifestyle advice.
  • Triage patients and decide on escalation or referral.
  • Provide patient education and follow-up care.
  • Contribute to service planning and improvement initiatives.
  • Support clinical audits and quality improvement projects.
  • Provide clinical leadership to junior staff.
  • Use data and insights to improve patient outcomes.
  • Maintain accurate patient records.
  • Ensure compliance with policies.
  • Participate in revalidation and professional development activities.
