Walsall Council's Integrated Care Services (ICS) seek an ASYE Social Worker (Adult Intermediate Care) to join a multi-disciplinary team working with adults, carers and families to maximise independence after hospital discharge.

You will undertake statutory duties under the Care Act and Mental Capacity Act and contribute to safeguarding adults. The role includes weekend/bank holiday rotations, with supervision and ongoing professional development opportunities.

Qualifications

• Social Worker registered with Social Work England.
• Experience safeguarding adults.
• Ability to work weekends and bank holidays on a rotational basis.

Responsabilités

• Support individuals to maximise independence after acute illness or hospital admission.
• Assist with hospital discharge and community rehabilitation.
• Provide crisis intervention and prevent hospital admissions.
• Work with carers, families and partner agencies to achieve positive outcomes.
• Undertake duties under the Care Act and Mental Capacity Act and align with the Professional Capabilities Framework.
