In this role, you will play a vital part in supporting individuals to maintain their independence and improve their quality of life. You will be responsible for assessing the needs of clients, developing and implementing care plans, and providing direct support. Your expertise will also include crisis intervention, advocacy, and connecting clients to relevant resources within the community. As part of an interdisciplinary team, you will collaborate with other professionals to ensure a holistic approach to care. The ideal candidate will have a strong commitment to social justice, an understanding of the challenges faced by diverse populations, and the ability to build trusting relationships with clients. Responsibilities

• Excellent interpersonal and communication skills, with the ability to build rapport with clients from diverse backgrounds.
• Demonstrated experience in conducting assessments and developing care plans.
• Crisis intervention experience and the ability to handle high-stress situations calmly and effectively.
• Proficient in record-keeping, report writing, and utilizing electronic case management systems.
