The Associate Practitioner will work as part of the SalusMedical Services multidisciplinary team, supporting the delivery ofhigh-quality, patient-centred care across both the Rapid HomeVisiting Service and the Salus | Aceso - the commercial arm of Salus Medical Services.

The postholder will play a key role in the routinefollow-up, monitoring, and management of patients with chronic and long-termconditions, supporting continuity of care and helping to reduce unplannedhospital admissions.

Working under the supervision of the Duty ParamedicPractitioner and alongside the highly regarded Home Visiting Service, theAssociate Practitioner will contribute to the holistic assessment and ongoingmanagement of patients in their own homes, care homes, and community settings.

Main duties of the job

The postholderwill provide routine follow-up, monitoring, and ongoing management of patientswith chronic and long-term conditions, helping to maintain stability, reducehospital admissions, and promote self-management.

Working under thesupervision of the Duty Paramedic Practitioner and alongside the Home Visiting Team, the role involves undertaking delegated clinical tasks such asobservations, phlebotomy, ECGs, wound checks, and health screening activities.

This role isideal for an experienced healthcare professional from an Ambulance Service oracute care background, looking to develop their practice within a supportive,multidisciplinary environment.

Job responsibilities

Key Responsibilities

Clinical Duties

  • Undertake routine follow-up appointments for patients following a paramedic home visit or intervention.
  • Support the monitoring and management of chronic and long-term conditions (e.g. hypertension, diabetes, COPD, heart failure) under agreed protocols and clinical supervision.
  • Conduct observations, phlebotomy, ECGs, urinalysis, wound checks, and other delegated clinical tasks.
  • Record and report changes in patient condition to the supervising Paramedic Practitioner or GP.
  • Participate in health promotion and self-management support for patients and carers.
  • Maintain accurate, contemporaneous clinical records in line with professional standards and organisational policy.
  • Contribute to multidisciplinary case discussions and follow up on agreed actions.
  • Support the delivery of commercial health services under the Salus | Aceso division, including health screening and wellness checks for corporate and private clients.

  • Work within clearly defined competency boundaries and refer appropriately for advice, guidance, or escalation.
  • Undertake training and development to build knowledge and capability within community care and long-term condition management.
  • Participate in clinical supervision, competency assessments, and annual appraisals.

  • Adhere to Salus Medical Services policies on confidentiality, safeguarding, and information governance.

Development and Support

You will be supported by experienced Paramedic Practitioners and the Duty Paramedic Practitioner, with access to supervision, mentorship, and ongoing professional development.

Where applicable, development through the Apprenticeship Scheme will be offered to candidates who are eligible and meet the relevant Apprenticeship standards.

Please Note: This role does not currently offer a progression route into Paramedicine, and this pathway cannot be supported within Salus Medical Services.

Person Specification
Experience
  • Experience of working within the Ambulance Service, or equivalent services delivery acute and urgent care
  • Strong clinical assessment and communication skills.
  • Ability to work autonomously within defined parameters of practice.
  • Understanding of long-term condition management principles.
  • Competent in basic clinical procedures (e.g. observations, ECGs, phlebotomy).
  • IT literacy and familiarity with clinical systems (e.g. EMIS).
  • Full UK driving licence and access to own vehicle.
  • Evidence of Continued Professional Development
  • Foundation degree or equivalent qualification in Health and Social Care (e.g. Assistant/Associate Practitioner qualification).
  • Experience working in a primary, community, or urgent care setting.
  • Experience supporting chronic disease reviews or community care visits.
  • Knowledge of NHS primary care pathways and integrated community services.
  • Previous involvement in quality improvement or audit projects.
Qualifications
  • GCSE grade A to C in English and Maths
  • Qualified to NVQ level 3 in Health and Social Care (or equivelant)
  • Qualified to NVQ level 4 in Health and Social Care (or equivelant)
